Discharge planning is the transition of patient care from a hospital setting to a home, primary care provider or community and effective discharge planning is critical in increasing patient recovery time as it is an integral part of patient care. Poor discharge planning implementation is associated with a variety of consequences for both the individual patient/family and the health system as a whole. High work motivation is needed by nurses with efforts and cooperation between various parties in the hospital, including facilitating nurses to obtain information about discharge planning, always collaborating with others, always following developments in science and technology for consideration in completing work, and discharge planning. used as a condition for the patient to go home. This type of research is descriptive analytic with a cross-sectional approach, using a Likert scale. This research was conducted in the inpatient ward of Helsa Cikampek Hospital with a total sample of 30 respondents. The results of the bivariate analysis test obtained a p value; 0.000 < 0.05, it can be concluded that there is a relationship between the work motivation of nurses and the implementation of discharge planning. From the risk factor analysis, the odds ratio (OR) is 9.500, meaning that high nurses' work motivation has a 9,500 times greater chance of implementing discharge pleaning when compared to low nurses' work motivation.
